Patient Assessment & Admission

• Assess and admit eligible patients for home health care services.

• Conduct comprehensive OASIS assessments in accordance with regulatory and organizational standards.

• Evaluate patient needs and determine appropriate levels of home health services.

Care Planning & Implementation

• Develop individualized plans of care based on patient assessments, physician orders, and clinical standards.

• Implement, monitor, and revise care plans to ensure high?quality, safe, and effective patient outcomes.

Skilled Nursing Care

• Provide skilled nursing services and comprehensive clinical assessments in the home setting.

• Perform and manage physician?ordered treatments including:

  • PleurX drain care

  • Finger-stick coagulation checks

  • Wound care and wound VAC management

  • IV therapy

  • Phlebotomy

  • Tracheostomy care

  • PEG tube care

  • Ostomy care

  • Telehealth monitoring and support

Teaching, Supervision & Counseling

• Educate clients and caregivers on disease management, medication administration, treatments, and safety.

• Provide supervision and clinical guidance to patients and family members to promote independence and self?care.

• Offer counseling to support patient understanding and participation in the care plan.

Interdisciplinary Collaboration

• Work collaboratively with members of the multidisciplinary home health team to ensure coordinated, patient?centered care.

• Communicate effectively with physicians, therapists, social workers, and supervisors regarding patient status and care needs.

Clinical Documentation

• Complete accurate, timely OASIS documentation in compliance with regulatory and agency standards.

• Maintain thorough and clear clinical records to support quality care and continuity of services.

Professional Skills & Problem Solving

• Utilize strong critical thinking and clinical judgment to identify patient needs and respond effectively.

• Address patient and caregiver concerns promptly, employing sound problem?solving strategies.

Additional Responsibilities

• Participate in required weekend and holiday rotations.

• Perform other duties as assigned to support optimal patient care and agency operations.
