Description
The Geriatric Emergency Nurses GENIE is a comprehensive role to help improve emergency department care and outcomes for older adults. The role requires NICHE and Geriatric ENA course completion at the time of hire or within 6 months. The incumbent must: be an experienced professional who knows how to apply theory and put it into practice; display an in-depth understanding of the senior patient population, and current specialty standards of care; possess strong problem-solving knowledge and solution-based skills. This role supports the nurse at the bedside providing the tools and knowledge to provide quality geriatric-specific care, to recognize atypical geriatric presentations of illness, and to help improve geriatric patient outcomes. This role also serves as a clinical resource and liaison for patients and their families and assists with navigating all aspects of the healthcare system. The GENIE will provide clinical education in regards to specialty treatment and liaisons with the patient's clinical service providers to address specific concerns. The incumbent must possess excellent customer service skills, incorporating CICARE into their daily practice, work with a high volume of patients and a customer base with high expectations. They must have the ability to support patients with a wide variety of clinical and non-clinical needs as well as have clear and concise communication skills to teach patients about complex, sensitive clinical issues. Under the direction of individual supervisor. Job requires self-direction.
Salary: $92,600 - $202,200 Annually
Qualifications
Assessment/Reassessment/Planning Participates in patient and bedside staff rounding in the emergency department. Completes geriatric specific patient screenings and functional assessment at the beginning of the shift. Conducts clinical surveillance to ensure appropriate referrals were completed as appropriate and escalates identified opportunities to appropriate personnel in order to meet the patient's needs. Identifies actual or potential patient safety risks. Collects information about potential discharges by review of information from the shift report, patient, family interview and collaboration with physicians and the health care team. Reassesses patient needs when clinically warranted. Identifies potential barriers to discharges and transfers. Involves the physicians, staff and other healthcare team members in the development of a plan to manage patient throughout transitions of care. Incorporates information about patient needs in planning. Patient Care Coordination Establishes and coordinates a supportive care plan with goals and interventions based on treatment and individual patient / service provider needs. Recommends changes or new processes to increase patient satisfaction in navigating across the healthcare system for services. Works closely with service providers, advocating and representing the patient with a focus on the patient's satisfaction with the clinical aspects of their treatment experience. Identifies and works to remove barriers to the plan of care as appropriate. Analyzes patient satisfaction reports and metrics regarding clinical treatment and patient understanding of their care plan. Pro-actively identifies problems in processes and clinical operations affecting patient satisfaction. Collaborates with clinical team to develop improvement processes to increase patient satisfaction. Coordinates clinical and non-clinical resources based upon patient's diagnosis, insurance coverage and community programs. Applies clinical knowledge to explain treatment options. Identifies patient's preferences, and any barriers to the treatment plan implementation. Collects, evaluates, and presents meaningful use and / or relevant operational or clinical data to leadership. Establishes recommendations for follow up and action plans for improvement in clinical areas. Assists in developing a culture oriented around patient satisfaction. Participates in relevant committees, task forces, coalitions, associations, and meetings to represent the clinic and the patients. May participate on teams for improvements in clinical applications, in conjunction with workflows. Liaisons with clinicians and departments, proposes improvements to workflows; facilitates improved coordination of care based on patient's treatment plan and each practice's scheduling requirements. Anticipates and initiates coordination activities; provides road maps for patients to set expectations and coordinate care in alignment with patient's needs. May include reviewing clinical case with provider team to schedule evaluation services. Knowledge of emergency medical services Strong organizational and interpersonal skills Ability to determine appropriate course of action in more complex situations Ability to work independently, exercise creativity, be attentive to detail, and maintain a positive attitude Ability to manage multiple and simultaneous responsibilities and to prioritize scheduling of work Ability to maintain confidentiality of all medical, financial, and legal information Ability to complete work assignments accurately and in a timely manner Ability to communicate effectively, both orally and in writing Ability to handle difficult situations involving patients, physicians, or others in a professional manner BSN required, MSN preferred Minimum 3 years experience in a hospital emergency department and 2 year recent acute care experience
