Overview
St. Luke's Health system is currently seeking a Post-Acute Care Physician to join our highly collegial and patient focused, multidisciplinary Post-Acute Medical Group. The selected individual will help to oversee two skilled nursing facility (SNF) sites as the facilities Medical Director as well as provide direct patient care. This opportunity boasts a great work/life balance. St. Luke's providers partner with several skilled nursing facilities in the Treasure Valley and Magic Valley to provide a level of care that exceeds traditional past practices in nursing facilities and provides on-site availability for when care demands are highest. The St. Luke's Post-Acute Medical Group provides timely evaluations to both our short-stay patients and our long-term care residents needing chronic care and work diligently to prevent visits to the emergency department or hospital readmissions.
The ideal candidate will be Family Medicine or Internal Medicine trained, preferably with experience working in an Acute Care setting (i.e.: Hospitalists or other inpatient).
