Nemours is seeking an Inpatient Rehabilitation Program Coordinator (Full-Time), for our Nemours Children's Health team in Orlando, Florida.

Inpatient Rehab Program Coordinator: Coordinating the referral/admission/discharge process for the inpatient rehabilitation program (IRP). Serving as the liaison to referral sources, utilization management, families, the rehabilitation team to insure seamless transition to the IRP at NCH. Works in collaboration with the internal and external case managers.
Provides evaluation, treatment, and consultation independently to patients across programs under the prescription of a physician within the guidelines designated by department and hospital policy and procedures. Demonstrates clinical competency in direct patient care. Position is mixed clinical and administrative.
- Coordinates the referral/admission process for the inpatient rehabilitation program with the Rehab Program Manager/Director, attending physicians, and the Transfer Center.
- Documents all required necessary insurance information in Epic for pre-registration including third party payer information, necessary medical documentation, contacts, and phone numbers.
- Facilitates the intake workflow processes between team members and in Epic including medical record management, coordination of insurance authorization, social work assessment, physician approval, bed planning, and admission.
- Provides rehabilitation program specific data for payer contract negotiations to Fiscal Services/Business Development Department as needed.
- Marketing the inpatient rehabilitation program to potential outside referral sources and collaborating with the referral sources to identify appropriate patients for admission to the program.
- Participates in the initial patient/family evaluation upon referral and follow-up contacts with patient/family and referring case manager to ensure smooth transition from referring facility to NCH and IRP. Coordinates and participates patient/family team conferences during patient admission and facilitates discharge planning needs including DME, medical supplies, home health services, transitions to other facilities, and follow-up medical appointments and therapy services.
- Evaluates neonates, pediatric and adolescent patients for rehab (physical, occupational or speech) including specialty areas such as splinting, positioning, adaptive equipment, feeding, home programs, home accessibility as applicable.
- Independently provides and coordinates age-appropriate rehab (physical, occupational or speech) treatment plans.
- Provides information, evaluation, treatment, and support to families while considering the lifelong implications for various diagnoses of the patient.
- Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee's supervisor.
Job Requirements
- Master's Degree required.
- Minimum of six (6) months to one (1) year experience required.
- State of Florida Therapy Licensure for Physical Therapy, Occupational Therapy, or Speech Therapy required.
- American Heart Association BLS Certification required upon hire.
- Pediatric rehab experience preferred.
