Overview

Responsibilities
Essential Functions/Responsibilities of Position:
- Home visits with home care patients, providing disease counseling, end of life discussions with goals of care, community resource referrals, assistance with applications to community resources, Bi-lingual a plus
Qualifications
Qualifications:
Minimum Education:
- Master's in Social Work from an accredited school of social work required
License/Certifications:
- NJ Social Work License LSW or LCSW required
Relevant Experience:
- Disease counseling and community resources
